A multi-disciplinary program to reduce cardiovascular risk factors through
physical exercise and educational sessions. Combines monitored and
supervised exercise along with extensive patient/family education. May be
repeated for credit. Prerequisites: Physician referral and permission of Department or Division. | |||

1. | Participate in follow-up activities after Phase I and/or II cardiovascular rehabilitation. (I) |
2. | Participate in patient/family awareness sessions about cardiovascular disease to promote self-care activities. (II) |
3. | Participate in monitored and supervised exercise to improve or maintain current physical capacity. (III) |
4. | Participate in a multi-disciplinary program about cardiovascular risk factor modification. (III,IV) |
5. | Participate in a program that enhances the "quality of life" of each participant. (V) |
