ICD Diagnostic Coding

International Classification of Diseases (ICD) diagnostic code assignment. Use of appropriate guidelines and federal, state, and local compliance standards for diagnoses. Identification of health provider documentation that supports code assignment.

Prerequisites: [HCC145 and (BIO160 or BIO201)], or permission of Program Director.

Describe the purpose and historical development of the International Classification of Diseases (ICD). (I)

2.

Differentiate among nomenclatures, classifications, vocabularies, and taxonomies, and describe the role of each in clinical coding systems. (I)

3.

Explain the concepts of patient privacy and confidentiality as they relate to diagnostic code assignment. (II)

4.

Describe the standards and impact of ethical coding. (II)

5.

Relate current local, state, and federal regulations to coding situations. (II)

6.

Describe the impact of coding non-compliance including fraud and abuse. (II)

7.

Describe the organization and format of the ICD code books. (III)

8.

Define and use ICD coding conventions including instructional terms and notations. (III)

9.

Locate appropriate diagnostic codes using the ICD alphabetical index and the tabular list. (IV, V)

10.

Assign and appropriately sequence ICD codes for diagnostic condition. (V)

Apply current diagnostic coding guidelines for ambulatory and outpatient services. (VI)

Use health record documentation including ancillary and medication records to select and support diagnostic codes. (VII)
